It is established a list of information required for the documents on higher education of state-standard format, including a junior specialist's degree. It means the following:

• Name of the document on higher education;
• Graduates` name and surname;
• The name of the university that issued certain document or in the case training occurred in separate structural unit of the institution - the name of the unit;
• graduation year;
• graduates` qualification consisting of information on assigned a degree of higher education, specialty and specialization, and in some cases - a professional qualification;
• Series (1 letter, 2 figures) and diploma registration number (6 digits) that is assigned to the document after the registration process in the Unified State Electronic Database on education, as well as the date of issue;
• the head or authorized person`s position who signed the document, his/her name, initials and signature, as well as print, sealed the signature;
• small State Coat of Arms of Ukraine and the inscription «Україна» and «Ukraine»;
• Information about the educational documents legalization or apostille procedure (if necessary).
